Insurance Information

At South Jersey Oral & Maxillofacial Surgeons, LLC we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. They are well informed and up-to-date.  You can reach our Patient Benefits Coordinator by calling 609-654-1300 ext 116.  Once your surgery is scheduled, a complimentary benefits analysis will be prepared for you and our coordinator will contact you one week prior to the date.

Please bring your insurance information with you to the consultation so that we can expedite reimbursement.

Our office participates with the following Insurance Plans:

• Medical Insurance Plans: Aetna, AmeriHealth, AmeriHealth Administrators, Horizon BC/BS, Horizon Blue, Independent BC/BC Personal Choice, Keystone Health Plan East, and Qualcare, Medicare
• Dental Insurance Plans: Aetna (PPO only), CIGNA PPO Dental, Delta Dental, Guardian, Horizon (traditional & dental option), MetLife, United Concordia (select plans), UnitedHeatlhcare Dental
